How to apply

Follow the steps below to apply for your certificate. Any questions? Take a look at our frequently asked questions or call.

Determine if you are eligible to apply

In order to be eligible to apply through Registry Connect, you must:

  1. need a certificate or document relating to an event that happened in Alberta
  2. not currently live in Alberta

If you are living in Alberta, please contact your nearest registry office.

As of April 1, 2016, clients wishing to request a photocopy of registration of an open record will need to apply directly to the Provincial Archives of Alberta. Please contact us if this applies to you.

Further eligibility requirements for each type of Vital Statistics document must also be met. These are listed on pages 2 and 3 of the application form.

Complete the application form

Pages 1 to 4 of the application form contain information that will help you complete your application. Please read these pages carefully.

On page 5 - Application for Certificate/Documents - complete all the information in the top section, then fill out the section appropriate to the type of certificate that you are requesting - Birth, Marriage, Death, or Legal Name Change.

If you have valid (not expired), government-issued photo identification:

  • On page 6 - Statutory Declaration for Proof of Identity - the Applicant must verify their own valid photo identification. To do this, the page must be taken to a Notary or Commissioner for Oaths, who must look at your identification, and then complete questions 1 through 7. Then the Notary/Commissioner must sign the form and provide their name and the expiry date of their commission. The Applicant must also sign the form.

    Please note Any changes made to your Statutory Declaration (crossed or whited out information, or information added later in a different colour pen) must be initialled by the Notary or Commissioner of Oaths who notarized your form. Changes that have not been initialled, or have been initialled by someone other than the Notary or Commissioner of Oaths, will result in a returned Statutory Declaration.

If you do not have valid government-issued photo identification:

  • You will need to appoint a Designated Agent to become the Applicant on your behalf. Your Designated Agent can be anyone, as long as they have their own valid government-issued photo identification, and as long as you have known them for at least 1 year. To authorize a Designated Agent, please fill out page 7 - Applicant's Consent to Designated Agent. Your Designated Agent now becomes the Applicant for the request, and your Designated Agent must fill out page 6 - Statutory Declaration for Proof of Identity - to verify their own identification (please see above).

On page 8, choose a service option and provide payment information (please see below).

Choose a service option

We offer three different service options:

  • Gold Service

    These rush requests receive top priority. Clients are contacted immediately by phone or email if there are questions or concerns with the application. Also, if there are discrepancies between what is on the application and what is on the original record, clients are immediately contacted by phone. Documents are then sent out by Purolator courier, and delivery is trackable.

    Cost: $39.64 per document, plus a $30 rush processing service fee, plus the courier's delivery fee which varies according to the destination.

  • Silver Service

    This service also offers priority processing. Clients are contacted immediately by phone or email if there are questions or concerns with the application. Also, if there are discrepancies between what is on the application and what is on the original record, clients are immediately contacted by phone. Documents are then sent out by regular mail via Canada Post, and are not trackable.

    Cost: $39.64 per document, plus a $30 rush processing service fee.

  • Bronze Service

    Regular processing time for certificate requests is approximately 1-3 business days in our office. We will contact you by email or letter about any issues with your application. Vital Statistics will communicate with you by letter regarding any discrepancies between what is on the application and what is on the original record. Documents are sent out by regular mail via Canada Post, and are not trackable.

    Cost: $39.64 per document (no other fees will apply).

Make your payment

We accept payments by cheque, money order, and credit card:

  • If you are paying for your request with a cheque or money order, please choose your service option on page 8 and include your payment with the rest of your application. Cheques or money orders are to be made out to Registry Connect, and must be drawn on a Canadian or US bank.
  • If you are paying by credit card, please make your payment online. We accept VISA, MasterCard, and Discover. Once your payment has completed, you will receive a transaction number - please write this number on page 8 of your application. You will also receive a receipt by email to keep for your records.

All fees are payable up front - there are no COD deliveries for certificates. Once an application has been processed, fees are usually non-refundable. For more information please see our refund policy.

To complete your application, you must still mail or courier your application form to us.
